For initial visit (without hospital card)

# Hospital card is common to all departments
of the hospital and is necessary for subsequent
visits to the hospital
Visiting with referral letters

Those carrying a referral letter from
other hospital or clinic are requested
to go the
‘Regional Medical Coordinating Office’ located to
the left side of the Main Hall.
Those wishing to get an appointment for the subsequent visit
are requested to ask at the counter of respective
department.
The appointment card will be issued together with the
receipt
at the Cashier Counter.
When changing the appointment is needed, telephone call
should
be made between 15:00 and 16:00 during weekdays.
For subsequent visit
1. Those with appointment go directly to the counter of the respective department
and submit the hospital
card.
2. Those without appointment go to the Revisit Counter(2)
in the Main Hall to get
the revisit card by the machine, and submit the card at the respective
department when revisiting.
3. Those who have lost or forgot the hospital card go to the Revisit Counter(2)
in the Main Hall.
After finishing with medical care at the department

1. Receive a file containing accounting sheet and
bills,
go to the Cashier Reception(3)
and wait in front of
the Cashier for call.
2. After payment, receipt, prescription/claim ticket
and appointment sheet are
given.
3. Those without the need to pay on the spot due
to
automatic payment by the
bank, public money or
insurance receive prescription/claim
ticket and
appointment
card at Cashier Reception(3)
# Health Insurance Certificate is checked
at the first hospital visit every month.
Please report to
the Revisit Counter (2) or Cashier Reception Counter (3)
when there is any
change in the Health Insurance Certificate.
# Automatic payment from your bank account
is available.
Please ask at the Cashier.
Prescription
1. All prescriptions are filled at pharmacies
outside the hospital.
2. Prescription/claim ticket should be detached
from the receipt at the
Pharmacy Counter of this hospital and receive the respective prescription
for outside pharmacy.
3. You are advised to send the prescription
in advance by FAX to the designated pharmacy,
so as to shorten waiting time.

# Limited departments
offer afternoon medical service

appointment. Please see the bulletin board.
2. Clinic is closed on Sunday, national holidays,
1st and 3rd Saturdays,
and New Years holidays
(December 31to January 3)
# Emergency patients are
always seen.
