1. Produce yours Health Insurance Certificate, Admission Certificate,
Hospital Clothes Sheet at Admission Desk on
the 1st floor.
2. Bring your seal 
Items requested to bring in on admission:
Bath towels, towels, underwear,
slippers, tissue paper, spoon and folk, toilet
things, washbasin, teacup, soap, shampoo,
etc.
# Make sure that you write your name
on your personal belongings
# Do not possess excessive sum of
money or valuables.
# Daily necessities are on sale in kiosk (in-house grocery store)
located on
the first floor of the main building.
1. Smoking is prohibited and no alcohol drinking allowed.
2. Consult with attending physician or nurse for outing or overnight
visit.
3. Consult with nurses about bringing in electric appliances.
4. Use public telephone in the hospital for telephone call. Incoming telephone
from
outside is conveyed through the nurse station from 09:00
to 20:00.
5. Mobile phone and PHS are prohibited because it may affect medical electric
monitors.
6. All lights in the sickroom are put off at 21:00.
1. Visiting hour is from 14:00 to 20:00.
2. Refrain from visiting during rest hours, physicianfs round hours and
dinner time.
3. Visitors are requested to check with the nurse station when visiting.
4. Avoid visiting inpatients with children to prevent infection.
5. To facilitate the delivery of incoming mail to you, make sure that
your address bears
the name of your ward and the number of your room.
1. The monthly hospital charges are claimed at the end of the month.
2. When dismissed from the hospital, pay your charges at the Cashier Counter
(4)-(6)
in the Main Hall.
3. When paying the charges from your bank account, money will be drawn
next month.
| Consultations and Advises |
1. Pharmacists will visit with you in the sickroom to explain and instruct
about your
medication.
2. Attending dieticians will visit your sickroom to
explain and instruct about your diet during
and after hospitalization.
3. Medial social workers will advise at your request
about your hospital payment, medical care after
dismissal or other personal problems related with
your disease.
Feel free to visit the General Consultation Room
at 1st floor in the Main Hall.
1. Smoking is prohibited in the hospital.
2. In service are JA Saku-Asama Branch, Beauty Parlor, restaurant/tea room
eIppuku-teifin the sides of the Entrance.
3. Kiosk (in-house grocery store) and ATM are in service back in the Main
Hall.
4. Public phones are located in the Entrance Hall, on each floor of the
ward.
Telephone cards are sold at the kiosk.
5. You can buy a TV ticket from one of the vending machines installed
in the hospital. (1,000yen/card)
6. Money changer is installed in the 1st floor Main Hall by the restaurant.
7. Charge for single-bed room a day is 3,150-10,500yen.
If you want a single room, please consult with
your attending physician or nurse.

1. You cannot leave your car in the hospital while you are in hospital.
2. Strictly refrain from giving money or other gifts to any staff hospital
member.
3. Suggestion boxes are located in several places in the hospital.
